1. Acceptance of Terms
By accessing or using this website (the "Site"), you agree to be bound by these Terms of Service. This Site is a joint marketing platform for two separate professional practices: A Corporation in California and a Sole Proprietorship in Hawaii. If you do not agree to these terms, please do not use the Site.
2. Separation of Legal Entities
You acknowledge and agree that professional services are provided by two distinct legal entities based on the patient's geographic location:
-
California Services: Provided by Magnolia Family Therapy Center, Inc., a California Professional Corporation.
-
Hawaii Services: Provided Magnolia Family Therapy Center.
These entities are legally separate. Neither entity is responsible for the professional, financial, or legal obligations of the other. Assets of the California Corporation are not available to satisfy liabilities of the Hawaii solo practice, and vice versa.
3. No Therapist-Client Relationship
The information on this Site is for informational purposes only. Use of this Site, including sending an inquiry through a contact form or email, does not create a therapist-client relationship. A professional relationship is only established after:
-
A formal initial consultation has occurred; and
-
A state-specific "Informed Consent for Treatment" and "Notice of Privacy Practices" have been signed by both you and the provider.
4. Telehealth & Jurisdiction
Services are provided via telehealth. To receive services, you must be physically located in a state where the provider is licensed at the time of the session:
-
California Residents: Governed by the laws of the State of California and the California Board of Behavioral Sciences (BBS).
-
Hawaii Residents: Governed by the laws of the State of Hawaii and the Hawaii Department of Commerce and Consumer Affairs (DCCA).
